Welcome to the website of Placidity Wellness, LLC (dba Scottsdale Serenity Rehab) (“we,” “our,” “the Facility”). These Terms of Service (“Terms”) govern your use of https://scottsdaleserenityrehab.com (“the Website”). By accessing or using this Website, you agree to comply with and be bound by these Terms, as well as applicable Arizona state laws and United States federal laws.
1. Medical Disclaimer
The Website is intended for informational and educational purposes only.
It does not provide medical advice, diagnosis, or treatment.
Use of the Website does not create a doctor-patient, therapist-patient, or provider-patient relationship.

The confidentiality of alcohol and drug use treatment records maintained by Scottsdale Serenity Rehab is protected by federal law (42 U.S.C. § 290dd-2) and 42 C.F.R. Part 2.
We may not disclose to any person outside the Facility that you are a patient, or disclose any information identifying you as having a substance use disorder, unless:
- You provide written consent,
- The disclosure is allowed by court order, or
- The disclosure is permitted by law (e.g., medical emergency, reporting child abuse or neglect, crimes committed on facility premises).
Federal law and regulations do not protect information about a crime committed by a patient either at the Facility or against Facility staff.
Violating confidentiality rules is a federal offense.
5. No Guarantee of Results
Recovery outcomes vary. Scottsdale Serenity Rehab makes no guarantees of recovery, sobriety, or treatment success.
Any testimonials or patient stories presented are individual experiences and not predictive of typical results.
6. Patient Privacy and HIPAA
Your health information is protected under HIPAA and Arizona medical privacy laws.
Any Protected Health Information (PHI) collected through this Website is handled in accordance with our Notice of Privacy Practices (HIPAA).
