Your Rights as a Patient

At Scottsdale Serenity Rehab, every individual is treated with dignity, compassion, and respect.
You have the right to:

  1. Respectful Care – To receive treatment free from discrimination, abuse, or neglect.
  2. Participation in Treatment – To be informed about your diagnosis, recommended care, and treatment options, and to participate in creating your treatment plan.
  3. Informed Consent – To be told about the benefits and risks of treatment before agreeing to it.
  4. Refusal of Treatment – To refuse or withdraw from treatment at any time, within the limits of law and program policy.
  5. Confidentiality – To have all personal and health information kept private in accordance with HIPAA and federal privacy laws.
  6. Access to Records – To request to see or obtain copies of your records, or request corrections to inaccurate information.
  7. Continuity of Care – To receive appropriate referrals if a different level of care is needed or our program cannot meet your needs.
  8. Safe Environment – To receive care in a safe, secure, and clean environment.
  9. Freedom from Restraint – To be free from unnecessary physical or mental restraint or seclusion.
  10. File a Complaint or Grievance – To voice complaints or grievances without retaliation or interference.

Your Responsibilities

As a participant in treatment, you are responsible for:

  • Providing accurate and complete information about your health and history.
  • Following your agreed-upon treatment plan to the best of your ability.
  • Treating staff and other clients with courtesy and respect.
  • Respecting facility policies, schedules, and safety rules.
  • Refraining from violence, harassment, or possession of drugs/alcohol on premises.
  • Meeting financial obligations and communicating any billing concerns.
  • Informing staff of changes in condition or medication.

If you ever have a concern or complaint about your care, privacy, or experience, please contact us so we can resolve it quickly.

What happens next:

  • You will receive acknowledgment within five (5) business days.
  • A full review and written response will be provided within thirty (30) days.
  • No retaliation or discrimination will occur as a result of filing a complaint.
